This is a yearly fee for registration that must be paid before the child arrives to the first day of class. Please fill out registration by clicking the link.

Ballet I Class (K-3rd) 3:00pm - 4:00pm on Tuesday's
One hour class, meets once a week.
Contemporary I Class (K-3rd) 2:30PM-3:30PM on Wednesday's
One hour class, meets once a week.
Contemporary II Class (4th-8th) 3:30PM-4:30pm on Wednesday's
One hour class, meets once a week.
- Baby Contemporary Class (2 - 3 Year old) 10:00am - 10:30am on Friday's
30 minute class, once a week.
Contemporary I Class (4-5 Year old) 10:30am - 11:00am Friday's
30 minute class, once a week.
Waiver Documents
General Liability
By registering my child(ren) with the Allegro Ballet Orlando, I agree to participate (or allow my child(ren) and family members to participate) in the Allegro Ballet Orlando, and hereby release Allegro Ballet Orlando, its directors, officers, agents, coaches, and employees from liability for any injury that might occur to myself (or to my child(ren) and family members) while participating in the Allegro Ballet Orlando program, including travel to and from training sessions or other scheduled organization activities. I agree to indemnify and hold harmless the above mentioned organizations and/or individuals, their agents and/or employees, against any and all liability for personal injury, including injuries resulting in death to me, my child(ren) and/or other family members, or damage to my property, the property to my child(ren) and/or other family members, or both, while I (or my child(ren) or family members) participating in the Allegro Ballet Orlando program and other classes.
Photo Consent
Parent Release Form for Photography and Videography I, the undersigned, give permission for Allegro Ballet Orlando to use video footage and / or photographs of my child/ward, This usage may include (but is not exclusive to) displaying publicly, distributing, or publishing, photographs, and/or video of my child for use in materials that include, but may not be limited to: - printed materials (eg - brochures and newsletters) - online and offline advertising and promotion - videos and digital images such for use on Social Media. By agreeing to this form, I acknowledge that I am giving unrestricted permission for my child’s image to be used in print, video, and digital media. I agree that these images may be used by Allegro Ballet Orlando for a variety of purposes and that these images may be used without further notification. I do understand that any identifying information including surname and location will not be used in conjunction with any video or digital images.
Medical
I certify that I am the parent or legal guardian for my child(ren). I hereby give my permission for any supervisor, coach or other team administrator associated with the Allegro Ballet Orlando to seek and give appropriate medical attention for our child(ren) in the event of an accident, injury, illness. I will be responsible for any and all costs associated with any necessary medical attention and/or treatment. I hereby waive, release and forever discharge Allegro Ballet Orlando from all rights and claims for damages, injury, loss to person or property which may be sustained or occur during participation in Allegro Ballet Orlando activities, whether or not damages or loss is due to negligence. I hereby acknowledge that my children is (are) physically fit and capable of participation in all activities.
